Outpatient clinic location
Mesa, AZ
Our outpatient wound care clinic is in Mesa, at 4838 E Baseline Rd, Bldg 2, Suite 113. Mesa is the operational hub of Arizona Wound Care Group — patients from across the East Valley travel here for outpatient services including Hyperbaric Oxygen Therapy (HBOT), wound debridement, Negative Pressure Wound Therapy (NPWT), cellular and tissue-based skin substitutes, advanced dressings, offloading casts for diabetic foot ulcers, and comprehensive vascular and diagnostic wound assessment.
Our home-based team serves Mesa patients in their homes, and our facility-based team rounds at Mesa-area skilled nursing facilities, assisted living facilities, and hospitals. Mesa is also home to a significant population of older adults and post-acute facilities, which is part of why we built the practice here.
Tempe, AZ
Tempe sits directly west of Mesa, and the drive to our outpatient clinic is short for most Tempe residents. For patients who can travel, the Mesa clinic offers the full set of advanced wound care therapies. For homebound patients, our home-based team visits Tempe homes for wound assessments, dressing changes, debridement, and ongoing treatment. We also work with Tempe-area skilled nursing and assisted living facilities on contracted or per-visit wound rounds.
Chandler, AZ
Chandler is south of Mesa, and we serve Chandler patients across all three service tiers. Many Chandler patients with chronic wounds travel to our Mesa outpatient clinic for HBOT and other advanced therapies. For patients with mobility limitations or living in residential care, our home-based team and facility-based team work across Chandler.
Gilbert, AZ
Gilbert is immediately south of Mesa and is one of the fastest-growing communities in the East Valley. We see a strong mix of patients across all three tiers — many Gilbert patients come to our Mesa outpatient clinic, and our home-based and facility-based teams work across Gilbert for patients and partners who need on-site care.
Scottsdale, AZ
Scottsdale sits to the north of the East Valley. Scottsdale patients who can travel come to our Mesa outpatient clinic. For homebound patients in Scottsdale or residents of Scottsdale-area facilities, our home-based and facility-based teams cover the area.
Apache Junction, AZ
Apache Junction is east of Mesa, and our home-based and facility-based teams routinely serve Apache Junction patients and partners. Patients who can travel to our Mesa clinic for outpatient services find the drive manageable from most of Apache Junction.
Queen Creek, AZ
Queen Creek is on the southeast edge of the East Valley. We serve Queen Creek with home-based and facility-based wound care, and patients who travel to our Mesa outpatient clinic come from Queen Creek as well.
