About

About Arizona Wound Care Group

A physician-led wound care group serving Mesa and the Phoenix East Valley through three care settings — our outpatient clinic, patients' homes, and on-site at facility partners.

Our approach

Proactive and reactive wound care.

Our approach is both reactive and proactive. Reactive care includes debridement, infection management, Hyperbaric Oxygen Therapy (HBOT), Negative Pressure Wound Therapy (NPWT), skin substitutes, and dressing changes.

Proactive care includes diabetic foot screenings, custom diabetic shoe fittings, patient and family education on nutrition and wound prevention, offloading strategies, and helping manage the underlying diabetes and circulation issues that drive chronic wounds in the first place.

Done together, this dual approach helps reduce avoidable hospitalizations and complications while delivering expert care when wounds do occur.

The model

How the three-tier model works

Wound care should meet patients where they are — not the other way around. That's why we built three distinct service tiers under one clinical team.

Outpatient clinic in Mesa

For patients who can travel to us, we offer the full set of advanced wound care therapies in one place — HBOT, debridement, NPWT, cellular and tissue-based products, advanced dressings, offloading casts for diabetic foot ulcers, and comprehensive vascular and diagnostic assessments.

Home-based wound care

Many of the patients with the most serious chronic wounds are also the patients who can't easily leave the house. Our licensed nurses and nurse practitioners visit patients at home for assessments, dressing changes, debridement, and ongoing treatment.

Facility-based wound care

Skilled nursing facilities, assisted living facilities, and hospitals can't always staff a dedicated wound care team. We round on-site to provide that expertise — with documentation that supports survey readiness and continuity of care.

A board-certified physician with wound care credentials serves as our Medical Director, setting clinical protocols across all three tiers, overseeing complex cases, and ensuring regulatory compliance.

The Medical Director's role is to keep clinical practice consistent across settings — from outpatient HBOT and debridement protocols to bedside wound rounds at SNFs, ALFs, and hospitals — and to manage the relationships with the referring physicians and facility partners we work with every day.

Who we work with

Patients, families, and clinical partners

Patients and families

Most of the patients we treat have chronic, non-healing wounds — diabetic foot ulcers, pressure injuries, venous leg ulcers, post-surgical wounds that aren't healing on schedule, and traumatic wounds. Most are referred by their primary care physician, a specialist, or a hospital discharge team.

Facility and clinical partners

We work with skilled nursing and assisted living facilities, hospitals, home health agencies, and managed care plans that need a dependable wound care partner.

Dignity

Care delivered with respect for the patient's day, family, and home.

Evidence

Protocols built on current wound care standards and physician oversight.

Coordination

We close the loop with primary care, specialists, and facility teams.

Access

Three care settings so patients aren't forced to travel to get specialty care.

Where we serve

Mesa and the Phoenix East Valley

Our clinic is in Mesa. Our home-based and facility-based teams travel across the Phoenix East Valley to reach patients and partners wherever they are.
